A woman who said her brakes failed as her sport utility vehicle slammed into a car was arrested Friday in connection with the fatal crash, Omaha police said.

Raysheena T. Samuel, 26, of Omaha was booked on suspicion of misdemeanor motor vehicle homicide.

Samuel's Chevrolet Suburban was heading down an Interstate 480 exit ramp at Martha Street on Feb. 3 when it struck a westbound Honda Accord, police said.

The driver, Kathryn L. Hardiman, 50, of Omaha, was killed at the intersection with 28th Street.

Police have said alcohol did not contribute to the cause of the crash.

They declined to say Friday if they believed Samuel's brakes were functioning properly at the time of the crash. They also declined to answer other questions about Samuel's actions.

A passenger in the Honda, Jason M. Newman, 30, suffered broken ribs and other internal injuries.

Neither Samuel nor a passenger in the Chevrolet was injured.
